New Patients

We belong to the Southern Primary Health Organisation (PHO). When you first arrive, we will ask you to fill out an enrolment form. To check if you are eligible to enrol, go to the Ministry of Health Website.  We will request your notes from your previous practice.

On enrolment you will be offered a free nurse’s appointment to introduce you to the practice and make sure we have all your details, recalls up to date, etc.

 

Making an Appointment

Consultations with the doctors and nurses are by 15 minute appointment. To make an appointment phone the office 443 0710 between 8.30am and 6.00pm. The earlier you contact us the more chance you have of being seen at your preferred time. Please let the receptionist know if your appointment is urgent, or if it requires a longer time for example; an insurance medical or excision. Arrive a little early and let the receptionist know you have arrived. If you are unable to keep your appointment please phone us as soon as possible. The doctors and nurses will do their best to see you on time but please understand that sometimes emergencies and other instances occur. You will be informed if there is a delay.

 

Non Attendance

If you are unable to attend an appointment, please give us at least two hours notice of cancellation.  A $20 fee will be charged for non attendance without notification.

 

Patient Fees

We expect payment for services on the day. Credit will only be given with prior approval. We are happy to accept cash, eftpos and internet banking, credit card or cheque. Automatic payments are also accepted:
