Long-term conditions (LTCs) can be defined as any ongoing, long term or recurring conditions that can have a significant impact on people’s lives.

LTCs include conditions such as diabetes, cancers, cardiovascular diseases, respiratory diseases, mental illness, chronic pain, chronic kidney disease and dementia. When these impact significantly on lifestyle, our trained nurses can offer support to manage these conditions as effectively as possible within the context of individual lifestyles.

As people age, this can become increasingly difficult and our nurses are committed to helping people stay in their own homes as long as they are able. When extra support is needed, our nurses will organise a comprehensive health assessment to identify how individual needs can best be met. This may include home visits and referrals to other services.

Our nurses work closely with our doctors and community providers such as home help agencies, physiotherapists and occupational therapists to ensure appropriate individualised packages of care. Our aim is to support people to self manage and live well in their own communities.

Leigh Cashen, our LTC Team Leader, has post graduate qualifications in Long Term Condition Management and leads a team of nurses with an interest in this area of nursing.
